Upon receipt of the order sheet, we would send you the confirmation.

Test Tool Supply Agreement is required for the first purchase of the products in the order sheet (*Except BD Blank Test Discs).
Please send two Agreements to us by courier, after reading, filling out completely and signing them.

The test tools are shipped upon.

• Receipt of one original copy of executed Test Tool Supply Agreement
AND
• Acknowledgement of receipt of payment to our account

The test tools should be shipped via courier. Please return the Acknowledgement of receipt by fax or e-mail.

If you would like to purchase the tools via company in Japan (your agent, etc.), please inform us beforehand.
